10.6.8 Recording The Assessment
Every RF risk assessment should be documented and linked to the applicable RF Radiation Safety Plan.
The assessment should record:
- its scope and the people and exposure categories considered;
- the RF sources, maximum operating states, work activities, and exposure scenarios;
- the direct, indirect, and secondary hazards considered;
- the data, assessment methods, assumptions, validation, and uncertainty;
- the results and comparison with applicable limits, including multiple-source summation;
- existing controls and evidence that they are effective;
- additional controls selected according to the hierarchy of controls;
- residual risk, access conditions, and any restrictions on the work;
- the people responsible for actions and the required completion dates; and
- approval, review dates, and event-based review triggers.
Clear documentation demonstrates the basis for the decision, communicates the conditions on which safe work depends, and provides a record for audits, changes, incident investigations, and later reassessment.
10.6.9 Reviewing The Assessment
An assessment is valid only while its source information, assumptions, work activities, exposure conditions, and controls remain current.
Review is required when transmitter power, frequency, antenna configuration, software-controlled modes, source ownership, site layout, occupancy, or procedures change; when temporary or additional sources are introduced; when controls fail; after an incident or near miss; or when standards or assessment information are updated.
Scheduled review and consultation with affected workers help confirm that the assessment still represents actual conditions and that the controls remain implemented and effective.
