IARC Monographs on the Evaluation of Carcinogenic Risks to Humans, Volume 102—Non-ionizing Radiation, Part 2: Radiofrequency Electromagnetic Fields

IARC Monographs Volume 102 evaluates the carcinogenic hazard to humans of radiofrequency electromagnetic fields. An expert Working Group met at the International Agency for Research on Cancer (IARC) in Lyon from 24 to 31 May 2011, and the full Monograph was published in 2013 as Non-ionizing Radiation, Part 2: Radiofrequency Electromagnetic Fields.

IARC is the specialized cancer research agency of the World Health Organization. Its Monographs Programme identifies whether an agent or exposure can cause cancer under some circumstances. It does not set RF exposure limits, prescribe compliance methods, or provide a quantitative estimate of cancer risk for a particular source, technology, or exposure level.

Purpose and Scope

Volume 102 addresses RF electromagnetic fields from 30 kHz to 300 GHz. Although public attention centered heavily on mobile phones, the evaluation concerns RF radiation as a physical agent and includes exposure from wireless telecommunications and other occupational, environmental, medical, industrial, broadcast, and radar sources.

The Monograph focuses on carcinogenicity. It does not evaluate every possible biological response or health outcome and should not be used as a general review of thermal injury, electrical stimulation, medical-device interference, or other non-cancer RF safety questions.

Evidence Reviewed

The Working Group reviewed exposure data, studies of cancer in humans, cancer bioassays in experimental animals, and other relevant data including mechanistic evidence. The published structure contains separate sections for these evidence streams followed by a summary and overall evaluation.

The epidemiological evidence included studies of wireless-phone use and tumors of the brain and acoustic nerve, including the multinational INTERPHONE study and a Swedish case-control series. Occupational and environmental exposure evidence was also considered, although the available studies differed substantially in exposure characterization, design, and informativeness.

The evaluation represents the evidence available to the Working Group by May 2011. Later animal, mechanistic, epidemiological, and exposure studies were not evidence used to reach the Volume 102 conclusion and must be assessed separately in any contemporary review or future IARC re-evaluation.

The Group 2B Evaluation

The Working Group classified radiofrequency electromagnetic fields as possibly carcinogenic to humans, Group 2B. Positive associations had been observed between wireless-phone exposure and glioma and acoustic neuroma, but chance, bias, or confounding could not be ruled out with reasonable confidence. The Monograph therefore judged the evidence in humans to be limited rather than sufficient.

Volume 102 also judged the evidence for carcinogenicity in experimental animals to be limited. Mechanistic and other relevant findings were reviewed as part of the overall evaluation, but they did not convert the evidence into a higher classification.

Group 2B means that a cancer hazard is possible on the evidence evaluated; it does not mean that RF exposure has been shown to cause cancer at ordinary environmental levels. It also does not quantify how large any risk might be, identify a safe or unsafe field strength, or imply that every agent in Group 2B presents the same risk.

Hazard Identification and Risk Assessment

Hazard identification asks whether an exposure is capable of causing cancer under some conditions. Risk assessment additionally asks how likely the outcome is at a defined exposure, taking account of magnitude, duration, frequency, route, population, and uncertainty. Confusing the two can turn a strength-of-evidence classification into an unsupported claim about a particular mobile phone, base station, workplace, or community exposure.

The Monograph is therefore relevant to health agencies, researchers, and guideline developers, but it is not a compliance standard. RF restrictions and reference levels are supplied by documents such as the ICNIRP Guidelines, IEEE exposure standards, and national requirements. Those bodies consider the wider health evidence and translate established adverse effects into quantitative restrictions.

Interpreting the Monograph

Volume 102 should be read with its evidential qualifiers intact. Limited evidence does not mean no evidence, but it also does not establish a causal relationship. The classification is broader than mobile phones, yet the principal limited human evidence arose from wireless-phone studies. Neither point should be omitted when the conclusion is summarized.

The 2013 publication date and 2011 evidence cutoff are also important. A later paper cannot retrospectively strengthen or weaken what the Working Group concluded from the evidence then available. Contemporary conclusions require a new systematic assessment that includes later research and applies the relevant review methods.

Significance for RF Radiation Safety

For RF radiation safety, the Monograph is a prominent example of formal hazard evaluation and weight-of-evidence reasoning. It shows how epidemiological, animal, mechanistic, and exposure evidence can be integrated while preserving uncertainty and distinguishing limited evidence from proof of causation.

Its practical message is precise: IARC classified RF electromagnetic fields as a possible cancer hazard, Group 2B, on the evidence evaluated in 2011. That conclusion supports continued research and careful evidence review, but it does not replace exposure standards, quantify risk for a particular exposure, or demonstrate that compliant RF exposure causes cancer.
